American Citizen Service Outreach Program in Qingdao, Shandong Province

A vice consul from the U.S. Embassy will be available on Thursday, April 10, 2008,  from 4:30 pm to 6:30 pm in the lobby of the Crowne Plaza Hotel (青岛颐中皇冠假日酒店) in the second floor lobby seating area. The address of the Crowne Plaza Hotel is No. 76 Xiang Gang Middle Road(青岛市香港中路76号) and phone number is 0532-85718888.

The vice consul will be available to assist American citizens by adding passport pages,  providing voting information, and providing registration information. Passport applications and absentee ballots requests will also be available, and the vice consul will answer any other questions related to Embassy services.   Unfortunately, we will not be able to accept any first time passport applications or provide notarial services.

If you wish to have additional pages added to your passport or renew your passport, you must email us the following information by close of business on Thursday, April 3rd: (1) name (as it appears in the passport); (2) date of birth; (3) gender; (4) place of birth; (5) social security number; (6) permanent address; (7) home and/or business telephone; (8) passport number; (9) passport issuance and expiration date and its place of issuance.  Our e-mail address is amcitbeijing@state.gov.

If you are requesting a paid service, please remember to bring exact change and provide an e-mail request beforehand.  Unfortunately, if you do not e-mail in advance we cannot provide additional pages or renew passports.

Please ask the hotel reception desk if you cannot locate the outreach program. We look forward to seeing you there.
