A vice consul from the U.S. Embassy will be available to meet with U.S. citizens in Qingdao on Thursday, October 29^th, from 12:30 to 1:30 p.m. at the Queens Hotel (青岛快通国际酒店).  The Queens Hotel is located at 89 Minhang Road Liuting International Airport, Chengyang District, Qingdao (青岛城阳区流亭国际机场民航路89号).  The hotel’s telephone number is 0532-84716777.  The vice consul will be available in the lobby of the hotel.

The vice consul will be available to assist American citizens by performing notary services, adding passport pages, accepting passport applications, providing registration information, and answering other questions related to Embassy services. 

If you wish to have any services performed, including notary services, passport renewals, or additional pages added to your passport, you must email us the following information by close of business on Monday, October 26, 2009.

(1) name (as it appears in the passport);

(2) date of birth;

(3) gender;

(4) place of birth;

(5) social security number;

(6) permanent address;

(7) home and/or business telephone;

(8) passport number;

(9) passport issuance and expiration date and its place of issuance.
