First Time Passport Issuance for Adults (ages 16 and above)

Please fill out the passport application form online and bring it with you on the day of your appointment.

Download the application form now!

  1. Proof of citizenship (eg. sealed birth certificate, consular report of birth abroad (Form FS-240), Certificate of Naturalization)
  2. Photo identification
  3. Two recent 2x2" (5x5 cm) color photos with white background, see photo requirements
  4. Complete form DS-11 (do not sign)
  5. USD 100 or RMB equivalent

Note: Please print out and submit only one-sided pages of the application.
