VISA FORMS

Prior to the interview, all applicants must fill out the electronic DS-156 application form.  Additionally, all applicants 16 years of age and older must complete the DS-157 application form.  Lastly, all Chinese applicants applying for F (Student), J (Exchange) and M (Non-academic Student) visas, and their dependents (F-2, J-2, M-2), must complete the DS-158 application form.

The DS-156 Electronic Visa Application Form must be filled out online.  Please print out all pages (including the bar code page-3 pages in total) and submit them at the time of the visa interview.  Glue one 2 X 2 inches color photograph to the online DS-156 form.  The photo should be less than 6 months old, have a white background, and be a full frontal view of your face.  Click for more information on Photo Requirements.

The DS-157 Supplemental Nonimmigrant Visa Application Form may be downloaded from your computer.  Type your answers in the space provided below each item and print out the completed form.   You do not need to fill out a Chinese DS-157, but if you need its Chinese translation to help understand the English form, click here.  (Note: 1. When filling out Item 3 “Full Name (In Native Alphabet)”, Chinese applicants should write down their full name in Chinese characters; 2. Chinese applicants must also fill out items 5, 6, 7, 12 & 17 in Chinese characters; 3. For H1 & L1 visa applicants who have children, please write down the children’s names, genders & nationalities on the upper right corner of the DS-157 form.)

The DS-158 Contact Information and Work History for Nonimmigrant Visa is filled out only by applicants applying for an F, J, M or an F, J, M Dependant visa.  Type your answers in the space provided below each item and print out the completed form.  You do not need to fill out a Chinese DS-158, but if you need its Chinese translation to help understand the English form, click here.

Lastly, if in the past, your visa process was subject to special processing or you have a technical background, please submit a detailed resume with your application.  A detailed resume starts with your University education, detailing the schools you have attended, the degrees you have received, all research, projects, publications, etc.  Detailing your professional work experience includes listing places of employment and responsibilities, projects, publications etc.
