DS-156 Nonimmigrant Electronic Visa Application All applicants applying for a U.S. nonimmigrant visa must complete the DS-156 Electronic Visa Application Form.

The online DS-156 form contains a bar code, which will allow us to electronically transfer data.  Once the application form is completed online, it must be printed, and then submitted at the time of the visa interview. Click HERE to learn how to fill out the online DS-156 form. 

For now, DS-157 and DS-158 application forms are still non-electronic.  They can be downloaded from this page and you can type your answers into the forms and print them out.  

DS-157 Supplemental Nonimmigrant Visa Application All applicants 16 years of age and older must complete the DS-157 application form.  Click to download the DS-157 form, type your answers in the space provided below each item and print out the completed form. Note: 1. When filling out Item 3 “Full Name (In Native Alphabet)”, Chinese applicants should write down their full name in Chinese characters; 2. Chinese applicants must also fill out items 5, 6, 7, 12 & 17 in Chinese characters; 3. For H1 & L1 visa applicants who have children, please write down the children’s names, genders & nationalities on the upper right corner of the DS-157 form. )  You don’t need to fill out a separate Chinese DS-157, but if you need its Chinese translation to help understand the English form, click here.  

DS-158 Contact Information and Work History for Nonimmigrant Visa Applicant All Chinese applicants applying for F (Student), J (Exchange) and M (Non-academic Student) visas, and their dependents (F-2, J-2, M-2), must complete the DS-158 application form, which is two pages. Click to download the DS-158 form, type your answers in the space provided below each item and print out the completed form.  You don’t need to fill out a separate Chinese DS-158, but if you need its Chinese translation to help understand the English form, click here.
